The Case Presentation of Psoriasis

CASE STUDY –

A 44 years old Muslim male patient appeared in the clinic with the previously diagnosed condition – Psoriasis; buy was not getting relief with different modes of treatment – so he opted for Homoeopathic Treatment.

PRESENTING COMPLAINTS – 

  • Scaly offensive eruption on both lower limbs below the knee and on the upper limb since 11 months.
  • Severe aching pain in both upper and lower limbs with greenish-yellow discharges.
  • < - walking, cold exposure, hanging limbs.
  • > - warm application.

HISTORY OF PRESENTING COMPLAINTS – 

  • It started on both legs as small black eruptions with discharge. Took allopathic treatment and condition became worse and eruption also appeared on upper limbs.
  • Skin broke with discharge of greenish-yellow color with bleeding, oedema and was unable to make a step due to pain.

ASSOCIATED COMPLAINTS – 

  • Aching Pain in lower back since 3 years – after having a fall on the back.
  • < - lifting weight, stooping.
  • > - lying down flat in supine position.

PAST HISTORY – 

  1. Jaundice – in childhood at 3 years of age.
  2. Chicken Pox – in childhood at 8 years of age.
  3. Fall on back – before 3 years.

TREATMENT HISTORY – 

  • Undergone Ayurvedic and Allopathic Treatment for Backache which gives timely relief.

FAMILY HISTORY

  1. Father died due to Stroke.
  2. Mother and Elder Brother are having Back Pain.
  3. No history of Skin complaints in Family members.

PERSONAL HISTORY – 

  1. Appetite – Good. Prefers warm foods and drinks. Easy satiety of stomach even after taking small quantity of food.
  2. Diet – Non – Vegetarian.
  3. Thirst – 8-10 glasses/day.
  4. Desires – Sweets.
  5. Aversion – Vegetables.
  6. Intolerance – Pulses causes distension.
  7. Bowels – Regular and satisfactory.
  8. Urine – No complaints with frequency of 5-6 times/day.
  9. Sweat – Decreased.
  10. Sleep – Decreased due to complain of pain.
  11. Dreams – No any.
  12. Thermals – Hot patient.

LIFE SITUATION

  • He is the 2nd among the 2 children. Born and brought up in Rajkot. He was not interested in studies, so educated up to 8th std only. Later on, started his own shop of artifacts. Currently, married and having 2 children.

MENTALS – 

  • Easily gets angry but later on becomes sad on scolding.
  • Anxiety about the future of children.
  • Anxiety about financial matters.

PHYSICAL EXAMINATION – 

  1. Built & Nourishment – Moderately built and nourished.
  2. Pallor – No conjunctival pallor.
  3. Cyanosis – No cyanosis.
  4. Clubbing – Absent.
  5. Oedema - Bilateral pedal oedema.
  6. Lymphadenopathy – No lymphadenopathy
  7. Respiratory Rate – 16/min.
  8. Pulse – 70/min.
  9. Blood pressure – 118/84 mm Hg.

Inspection – 

  1. Colour - Greyish white.
  2. Size – Varying.
  3. Consistency – Seem to be hard.
  4. Configuration – Irregular.
  5. Margination – Ill-defined.
  6. Surface – Dry scales.
  7. Bilateral pedal oedema present.

Palpitation – 

  1. Rough, Thick, Dry surface.
  2. Bilateral pedal oedema present.

ANALYSIS AND EVALUATION OF SYMPTOMS – 

Mental Generals – 

  1. Easy get angered and get sad on scolding.
  2. Anxious about children.
  3. Anxious about financial matter.

Physical Generals – 

  1. Prefers warm foods.
  2. Easy satiety even taking small quantity of food.
  3. Prefers warm drinks.
  4. Desires – Sweets.
  5. Aversions – Vegetables.
  6. Intolerance - pulses causes distension.
  7. Sweat – Decreased.
  8. Hot Patient.
  9. Sleep – Decreased.

Physical Particulars – 

  1. Scaly offensive eruption on both lower limbs. < - walking, cold exposure, hanging limbs. > - warmth.
  2. Aching type of pain in low back. < - lifting weight, Stooping. > - lying flat.

TOTALITY OF SYMPTOMS – 

  1. Easily get angered and get sad on scolding.
  2. Anxious about children.
  3. Anxious about financial matter.
  4. Easy satiety.
  5. Desires – Sweets.
  6. Aversions – Vegetables.
  7. Intolerance - Pulses causes distension.
  8. Sweat – Decreased.
  9. Hot Patient.
  10. Sleep decreased.
  11. Scaly offensive eruption on both lower limbs. < - cold exposure, walking, hanging limbs; > - warm application.
  12. Aching type of pain in low back. < - lifting weight, stooping; > - lying flat.

Here, considering the patient as whole with his presenting symptoms, dominating miasm appears to be “Psora-Syphilitic”.

RUBRICS SELECTED – 

  1. Mind-Anger-Quick Repentance.
  2. Mind-Anxious-Children.
  3. Stomach-Easy Satiety.
  4. Generalities-Desire-Sweets
  5. Generalities-Aversion-Vegetables.
  6. Eruptions-Discharge-Moist.
  7. Eruptions-Painful.
  8. Eruptions-Scaly.
  9. Perspiration-Odour-Offensive.
  10. Generalities-Heat-Sensation of.

With the guidance of Synthesis Repertory and Various Materia Medica’s, finally Sulphur was selected to be the similimum remedy.

  • So, Sulphur 30 – Single dose was prescribed along with Placebo TDS for 1 month.
  • Also, patient was advised to keep the areas clean and hygienic.

1st FOLLOW-UP AFTER 1 MONTH – 

  • Offensive discharges were completely absent, but the eruptions were same – scaly, dry, rough skin.
  • Backache was as it is.
  • All-over, patient was relaxed. He was having a general sense of happiness.
  • Hence, to remove the skin complaint and backache again, Sulphur 30 – Single dose was prescribed along with Placebo TDS for 1 month.

2nd FOLLOW-UP AFTER 1 MONTH – 

  • Scaly eruptions reduced, cleared was appearing gradually.
  • Backache slightly reduced, but still was having complain in weight lifting.
  • So, Placebo was prescribed TDS again for 1 month.
