Case Presentation of Androgenic Alopecia treated with Constitutional Remedy

Case Study - 

A 27-year-old male patient came with complaint of hair fall since 5 years. Sudden onset and gradual progression. Hair fall is more while combing and after hair wash. There is baldness all over the head, fall of eye brows, moustache, and beard. Hair fall also worsens during winter and cold season.

Also, complaints of bilateral lower leg varicose veins, pain while standing for long time.

PRESENTING COMPLAINTS – 

  • Hair falls from scalp (mainly vertex region) since 5 years.
  • Baldness all over the head.
  • < - while combing, after hair wash, winter, cold season.

HISTORY OF PRESENTING COMPLAINTS – 

  • Suddenly it started with falling of hair on the vertex region.
  • Then with the gradual progression, it spread all over the head.

ASSOCIATED COMPLAINTS – 

  • Varicose veins and pain in both lower limbs < - long standing.

PAST HISTORY – 

  1. Diarrhoea (Recurrent) – in childhood at 2.5 years of age.
  2. Malaria – in childhood at 7 years of age.
  3. Fall on stairs – before 6 years.

TREATMENT HISTORY – 

  • Undergone Ayurvedic and Allopathic Treatment for fall injury.

FAMILY HISTORY

  1. Father having Hypothyroidism.
  2. Mother and Elder Brother are having Headache.
  3. No history of Skin and hair complaints in Family members.

PERSONAL HISTORY – 

  1. Appetite – Increased. Prefers warm foods and drinks. Easy satiety of stomach even after taking small quantity of food.
  2. Diet – Vegetarian.
  3. Thirst – Increased, cold water 3 – 4 litres/day.
  4. Desires – Spicy and baked food, Cold drinks.
  5. Aversion – Coffee.
  6. Intolerance – Coffee which causes distension of abdomen & nauseating feeling.
  7. Bowels – Regular and satisfactory.
  8. Urine – No complaints with frequency of 5-6 times/day.
  9. Sweat – Profuse on hands and feet, cold too touch.
  10. Sleep – Sleeps late, feels sleepy &unrefreshed in early morning.
  11. Dreams – No any.
  12. Thermals – Hot patient.

LIFE SITUATION

  • He is the eldest among the 2 children. Born and brought up in Rajkot. He was interested in studies and ranked in schools. He graduated in Engineering. Later on, started job in the company. Currently, still deciding for the marriage.

MENTALS – 

  • Patient is very jovial and speaks without hesitation.
  • He is an engineer by profession and maintains good relation but doesn't have close relationship with anyone in work area.
  • He is not close to even his family members. He doesn't want take responsibility of family and delayed getting married.
  • He also says that he gets sexual thoughts often even in public places. He doesn’t show much interest in his job & feels lack of interest in job & forgetfulness. 

PHYSICAL EXAMINATION – 

  1. Appearance – Lean, sunken face.
  2. Built & Nourishment – Moderately built and nourished.
  3. Pallor – No conjunctival pallor.
  4. Cyanosis – No cyanosis.
  5. Clubbing – Absent.
  6. Oedema – Sometimes pedal oedema.
  7. Lymphadenopathy – No lymphadenopathy
  8. Respiratory Rate – 18/min.
  9. Pulse – 76/min.
  10. Blood pressure – 128/74 mm Hg.

Inspection – 

  1. Size – Large area covered.
  2. Margination – Ill-defined.
  3. Surface – Shiny.
  4. Baldness.
  5. Mouth – Hygiene maintained.
  6. Tongue – Cracks present.

DIAGNOSIS Androgenic Alopecia.

ANALYSIS AND EVALUATION OF SYMPTOMS – 

Mental Generals – 

  1. Forgetfulness.
  2. Indifference.
  3. Sexual thoughts.

Physical Generals – 

  1. Desires – Spicy food, Cold water.
  2. Aversion – Coffee.
  3. Perspiration – Palm.
  4. Hot Patient.

Physical Particulars – 

  1. Hair fall.
  2. Tongue – cracked.
  3. Varicose veins.

TOTALITY OF SYMPTOMS – 

  1. Forgetfulness.
  2. Indifference.
  3. Sexual thoughts.
  4. Desires – Spicy food, Cold water.
  5. Aversion – Coffee.
  6. Perspiration – Palm.
  7. Hot Patient.
  8. Hair fall.
  9. Tongue – cracked.
  10. Varicose veins.

Here, considering the patient as whole with his presenting symptoms, dominating miasm appears to be “Psora-Syphilitic”.

RUBRICS SELECTED – 

  1. Mind-Indifference.
  2. Mind-Forgetfulness.
  3. Mind-Lasciviousness.
  4. Mouth-Cracked-Tongue fissured.
  5. Generalities-Food and drinks-pungent things-desire.
  6. Generalities-Food and drinks-cold drink, cold water-desire.
  7. Generalities-Food and drinks-coffee-aversion.
  8. Extremities-Perspiration-Hand-palm.
  9. Head-Hair-baldness.

PROVISIONAL REMEDY – 

Fluoric acid –

  • Lascivious thoughts.
  • Forgetfulness.
  • Fear of responsibility.
  • Indifference.
  • Jovial nature.
  • Desire – spice, cold things.
  • Perspiration – palm and soles- cold and clammy.
  • Thermal – hot patient.
  • Baldness, varicose vein, cracked tongue, frequent history of tonsillitis.
  • Appearance lean.

With the guidance of Synthesis Repertory and Various Materia Medica’s, finally “Fluoric acid” was selected to be the similimum remedy.

  • So, Fluoric acid 200 – 1 dose/week was prescribed along with Placebo TDS for 1 month.

1st FOLLOW-UP AFTER 1 MONTH – 

  • Falling of hair reduced.
  • No itching, no dandruff.
  • Re-growing of hair.
  • Hence, Fluoric acid 200 – 1 dose/week was prescribed along with Placebo TDS for 1 month.

2nd FOLLOW-UP AFTER 1 MONTH 

  • Re-growing of hair.
  • No hair falls.
  • Forgetfulness reduced. Pt. feels better.
  • Hence, Fluoric acid 1M – 1 dose stat was prescribed along with Placebo TDS for 1 month.

3rd FOLLOW-UP AFTER 1 MONTH 

  • Re-growing of hair.
  • 75% improvement.
  • Indifference reduced. Marriage decided.
  • Forgetfulness reduced. Pt. feels better.
